Sarasota Memorial Health Care System: DECLINATION OF INFLUENZA VACCINATION

My friend is employed as a health care worker here in Sarasota. She recently received the following form,

"Declination of Influenza Vaccination."

She intends to decline the vaccination, in part because it is untested and uninsurable.  She also visits patients in their homes; at least one of her patients suffered an apparent adverse reaction after receiving the regular, "seasonal" flu shot a few weeks ago.
